Care Coordinator 

Position Title: Care Coordinator 

Department: Clinic

Hours: Part-Time / Flexible Hours

Job Summary:

The Care Coordinator will be instrumental in coordinating patient care, managing patient referrals and scheduling appointments. The Care Coordinator will develop and implement care plans, provide support in navigating necessary services and bridge any care gaps identified by our treatment teams to optimize care utilization and improve established patient outcomes. This position will work closely with providers and other clinical staff to streamline care processes, improve patient outcomes, and ensure that services are accessible and effectively managed. This individual will also participate in programs within the facility and produce reports as necessary.  

Duties/Responsibilities:

  • Observes patients, charting and reporting changes in conditions such as adverse reactions to medication or treatment and takes necessary actions.
  • Answers patients’ calls and determines how to assist them.
  • Measures and records patients’ vital signs such as height, weight, temperature, blood pressure, pulse, oxygen saturation and respiration.
  • Provides basic patient care or treatments.
  • Works as part of a health care team to assess patient needs, plans and modifies care, and implements interventions.
  • Supports nursing care standards through adherence to existing policies and procedures.
  • Obtains and documents nursing history from patients and/or significant others that is comprehensive of present condition and prior problems. Performs basic assessment of patients, indicating adequate knowledge of anatomy, physiology, and pathology. Records vital signs, notes changes, and pursues more-specific investigations as needed. Identifies abnormal diagnostic data and shows a fundamental understanding of its implications by reporting information to the provider and others caring for the patient, and by incorporating information in the nursing care plan.
  • Reviews and develops care plans for assigned patients. Includes the patient and his or her family in planning care, establishing priorities, and setting goals.
  • Uses care plans as a basis for providing safe and therapeutic care to patients. Communicates with the patient and his or her family and other members of the health care team in carrying out the goals of the care plan. Accepts accountability for clinical care of assigned patients including supervision of non-professional personnel.
  • Evaluates and notes the patient's response to nursing care defined in the nursing care plan and medical records.
  • Assumes responsibility for professional growth by evidence based practices and attendance at educational programs and facility workshops.
  • Instill confidence in patients by maintaining a neat appearance and performing duties in a competent and caring manner. Reports observations of patients to the nurse caring for the patient to ensure professional assessment.
