Case Manager in Lancaster, SC
Coordinates access to all services available to persons with disabilities including those available to Medicaid receipents & other necessary services within the community. Develops Initial Plan of Service for new referrals entering the DSN system. Completes an Individual Needs Assessment on an annual basis, semi-annual review of the Needs Assessment & updates as needed. Assists individuals in obtaining needed services and coordinates services from multiple agencies that are required to meet the individual's needs. Attends public school meetings, community support meetings & meetings with many other organizations on behalf of the individual. Serves as the individual's advocate, ensuring the individual's access to a wide range of services. Provides assistance in crisis intervention. Assures that all personal rights of the individual are maintained, including freedom of choice, civil and legal rights. Arranges family support servies.
