COMMUNITY HEALTH WORKERS
LMAS District Health Department is accepting applications for two Community Health Workers (CHW’s), one for Alger/Schoolcraft Counties, and one for Luce/Mackinac Counties. The CHW will facilitate needed health and human services for pregnant mothers/infants with Substance Use Disorders to secure better health outcomes. CHW's are valuable to their clients by being culturally and geographically connected to the communities they live and work in. Through home visiting in partnership with the Maternal Infant Health Program (MIHP) and Healthy Families UP, the CHW links clients to community resources, and assists them in overcoming barriers to substance use, health, social services, education, and employment.
Possession of a high school diploma or equivalent is required, CHW credentials and experience with social services or public health is preferred. Possession of a valid MI driver's license, reliable transportation, and ability to travel is required. Excellent customer service skills, knowledge or maternal/infant health, and knowledge of core functions and essential services of public health is a plus.
