Positions:

Physician and Physician Assistant/Nurse Practitioner

Seeking experienced PCP to join our team. This position provides the full scope of primary care services to our patients, including but not limited to preventive care, acute care, diagnosis, treatment and care coordination, with a daily patient load of approximately 20 patients per day. Full-time, 5 days per week, on-call and Saturday a.m. rotation. Schedule flexibility available if provider can cover every other Saturday, full day shift.

LCSW-R

Seeking additional LCSW-R to provide integrated behavioral health services in the primary care setting. Candidate will support patients through in-office counseling and/or help with referrals to appropriate specialists and community services. Overall goal is to maintain/enhance mental, emotional, behavioral, and social functioning of patients. Minimum 2 years’ experience in social work in an ambulatory setting, knowledge of available community resources and the ability to evaluate and make appropriate referrals required.
