Population Health Coordinator

Supervision: This position does not supervise any other staff
Summary: The Population Health Coordinator is responsible for helping our members attain optimal health and lead fuller lives. The Population Health Coordinator assists with coordination of intra- and inter-departmental
population health and health equity strategies and activities, as well as developing and supporting population health, health equity, and community development initiatives. The Population Health Coordinator takes an active role in developing and executing PrimeWest Health’s health promotion
efforts, supporting the PrimeWest Health Community Reinvestment Program (CRP), working with community organizations, and participating in community-organized health improvement initiatives. This position also assists with the development, implementation, and maintenance of population health programs and with regulatory compliance responsibilities, including regulatory reporting.
A. Reporting Accountability
This position is responsible for meeting the following regulatory, contractual, or other requirements on behalf of PrimeWest Health:
1. Population health management programs and projects, audits, and compliance reports for the Minnesota Department of Human Services (DHS) and the Centers for Medicare & Medicaid Services (CMS)
2. Regulatory reports assigned to the Population Health & Community Development team
3. National Committee for Quality Assurance (NCQA) requirements
4. CRP progress and outcome reporting, including grantee reporting and compliance oversight
B. Role and Key Responsibilities
1. Participate and contribute to all activities designed to improve our population health and health equity activities to help PrimeWest Health achieve our 4D objectives by routinely assessing the effectiveness of these activities and by following NCQA population health management models and standards, keeping apprised of the latest research and best practices nationally and statewide in the areas of population health management, health equity, and social determinants of health (SDOH).
2. Support and implement community organization efforts and development initiatives at the local level with targeted member populations and communities in need within the PrimeWest Health service area, including engaging local health care providers, human services providers, and community resource agencies in order to support and collaborate on local PrimeWest Health population health improvement efforts
3. Continually analyze PrimeWest member population health data for opportunities to improve population health and health equity and address SDOH
4. At the direction of the Population Health & Community Development Manager, communicate initiatives to relevant staff. Provide staff with the tools, training, education, direction, and motivation to perform their responsibilities to support initiatives and provide quality outcomes for members.
5. Update PrimeWest Health policies and procedures related to population health management and health equity, including annual review
6. Participate in measuring and evaluating the progress of member population health
7. Communicate and coordinate Population Health & Community Development activities and initiatives with other PrimeWest Health departments
8. Maintain positive working relationships and regular communication with county Public Health and Human Services agencies and health care providers to ensure their active support for and coordination with PrimeWest
Health’s population health improvement and health equity efforts
9. Assist with and ensure annual updates to the PrimeWest Health Quality Plan are completed in compliance with regulatory and contractual requirements and current community standards for quality activities
10. Participate in the PrimeWest Health Quality & Care Coordination Committee
11. Maintain and protect members’ protected health information (PHI)
12. At the direction of the Population Health & Community Development Manager, serve as Population Health & Community Development representative for interdepartmental discussions

7. Carry out other responsibilities and activities as needed and assigned by the Population Health & Community Development Manager or as agreed to as part of collaborative efforts and activities involving PrimeWest Health

D. Professional Competencies
1. Knowledge of and skills in population health management, community organization and development, health promotion, culturally competent care, care system navigation, health coaching, care coordination, case
management, and/or chronic condition management and population health management, including the concepts
embraced by the NCQA population health management model
2. Experience working in health and/or human services in areas with diverse population groups (race, ethnicity, language, income level, age, gender identification, sexual orientation) and with health care, human services,
and/or community SDOH resource agencies that provide care to those populations
3. Knowledge of NCQA population health management model and standards and applicable Federal and State of Minnesota laws, rules, and regulations governing health services provided to members through Medicare, Medicaid, and managed care programs
4. Experience working with diverse populations (race, ethnicity, language, income level, age, gender identification, sexual orientation) and providing culturally appropriate care
5. Ability to perform health care data analyses, plan and organize, promote member and family autonomy, maintain confidentiality, and maintain files and records
6. Comfortable with traveling and driving alone by automobile to remote rural Minnesota locations
7. Skills in group development and facilitation, community organization and development, health promotion, analyzing data, public speaking, and continuous quality and process improvement
8. Experience in organizational leadership, conflict management, and handling difficult people
9. This position requires a high degree of initiative, self-motivation, creative and analytical thinking, and independent decision-making. It also requires the ability to work with diverse populations in a variety of settings
E. Educational Requirements and Required Qualifications
1. Bachelor’s degree or higher in one or more of the following areas: community health, public health, health education, health promotion and prevention, sociology, social work, psychology, and/or mass health
communications. Nursing degree preferred.
2. Practical experience in one or more of the following areas: population health management, health promotion and prevention strategy and campaign development/implementation, community health, public health, social work, complex or chronic condition population management
3. Excellent written and verbal communication skills, including public speaking
4. Experience in analyzing and utilizing data for continuous quality, process, program, and strategy improvement
5. Work experience in management or employee supervision
6. Demonstrated knowledge or experience in providing culturally competent care and working with culturally diverse populations
7. Demonstrated knowledge of, and proficiency in, Word, Excel, and Outlook
8. Flexibility and ability to handle multiple tasks. Demonstrated organizational skills and attention to detail.
9. Ability to work effectively with a variety of people both within and outside of the organization
10. Ability to sit at a desk or computer station most of the time. Ability to occasionally lift and/or carry such articles as file folders and small boxes up to 20 pounds. Some travel, including overnight travel, required.
11. Ability to travel approximately 20 percent of the time.
