Job Title: Bilingual Behavioral Health Specialist

Reports To: Behavioral Health Program Manger

FLSA Status: Exempt

Schedule: Part-time and Hybrid (25 hours/week)

The Phoenixville Free Clinic (PFC) is a healthcare clinic serving the Greater Phoenixville Region. Our mission is to provide quality healthcare to the uninsured in an atmosphere that fosters dignity and respect for our patients. PFC serves more than 1,700 patients per year, employs a team of full-time and part-time professionals, and utilizes volunteers in healthcare and public health to ensure we are meeting the needs of the community. PFC recently developed and implemented a holistic Behavioral Health Program to address the critical mental health needs of our community and to improve health outcomes for our patients. This new service will integrate with our primary care and specialty services to support individuals, couples, and families. We are looking for someone to join our team as a part-time Behavioral Health Specialist, assisting the Program Manager and seeing patients. The Behavioral Health Specialist will provide individual, couples, family and group therapy to a diverse population. They will report and assist the Behavioral Health Program Manager/LCSW with program development and evaluation.

Responsibilities: • Consult with PFC primary care providers to enhance understanding of the patient, provide decision support for treatment planning, and assist in the implementation and monitoring of biopsychosocial treatment plans. • Perform individual, couples, and/or family consultation with patients including: • Functional and strength-based assessments and diagnosis. • Psychoeducation for patients and their support systems. • Medication adherence counseling and disease self-management counseling. • Motivational interviewing to develop behavioral strategies aimed at symptom reduction. • Problem-solving cognitive intervention aimed at modifying negative thinking and promoting self-efficacy. • Self-care plan development and skills training to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and relaxation. • Evaluation and identification of maladaptive coping strategies and development of harm reduction strategies. • Provide close monitoring and population health management of patients through follow-up contacts and coordinated services with other providers (internal and external) as needed. • Develop discharge plans with formal and informal recommendations and referrals. • Complete all charting as needed and required, including assessment and progress notes. • Maintain knowledge of resources and community partners available within our service area.

Qualifications: • Master's degree and licensure in social work, or a behavioral health related field preferred. • Bi/Multilingual – Portuguese/Spanish/English preferred. • A minimum of three years clinical experience preferred. • Proficiency in evidence-based treatment approaches (Problem-Solving Treatment, Motivational Interviewing, Cognitive Behavioral Therapy, and Behavioral Activation. • Knowledge of/experience with behavioral health tools (PHQ9, GAD7, ACES, etc.). • Experience with pediatrics (ages 6 +) and adults. • Strong organizational skills. • Excellent written and verbal communication skills. • Community based experience desirable. • Technical proficiency in a variety of platforms including EMRs, ability to quickly learn new technology.

Schedule: • Part-time, 25 hours/weekly, some flexibility (potential some early evenings and weekends), various locations in Phoenixville, some remote/telehealth.
